From San Juan County

To better understand the current need for vaccinations for San Juan County residents 65 and older, San Juan County Health and Community Services asks that those who meet all of the four measures below fill out a simple form:

  1. A San Juan County resident (short term or long term), and
  2. Age 65 or older, and
  3. Currently unvaccinated, and
  4. Interested in being vaccinated.

The form is available online here.
